• Conduct thorough psychosocial assessments to evaluate patients' emotional and social needs, ensuring a comprehensive understanding of their circumstances.
• Provide individualized and group counseling to address various issues, including adjustment difficulties, grief, relationship challenges, and coping strategies.
• Work closely with medical teams to develop integrated treatment plans that address both physical and psychological health.
• Coordinate with community organizations to facilitate post-discharge services, ensuring patients have access to essential resources.
• Offer education and emotional support to patients and their families regarding treatment options, available resources, and strategies for improvement.
• Participate actively in hospital committees, contributing your social work insights to foster interdisciplinary collaboration.
• Conduct crisis intervention and provide necessary support during challenging situations, facilitating a smoother healthcare journey for patients.
• Create and implement discharge plans that ensure continuity of care and connection to appropriate community services.
• Maintain accurate clinical documentation in compliance with healthcare regulations and organizational policies.

In this role, you will engage with a diverse patient population, including individuals experiencing mental health disorders, substance use challenges, and complex psychosocial situations. Your contributions will help address barriers to care, improve overall quality of life, and assist patients through transformative phases of their healthcare journeys.

• A Master's degree in Social Work from an accredited program.
• An active state licensure as a Licensed Independent Social Worker (LISW) in the state of Michigan.
• Strong knowledge of social work principles, medical terminology, mental health concepts, and crisis intervention methods.
• Excellent communication, interpersonal, and collaborative skills, essential for effective patient interaction and teamwork.

While a minimum of two years of experience in a healthcare or hospital setting is preferred, we welcome candidates with a strong passion for patient advocacy and a willingness to learn.
