Job Title: Lead Psychiatrist - Remote
Status: Part-Time

We deliver child welfare-informed, high-touch care to Medicaid-covered youth navigating foster care, adoption, kinship placements, and juvenile justice. Our model prioritizes consistency, trust, and accessibility in systems where they are too often missing.
Role Summary
The Founding Lead Psychiatrist is a fractional clinical leadership role responsible for helping design, launch, and scale psychiatric services at Here Now Health. During the initial phase, this role will focus on partnering closely with clinical leadership, operations, and product to build the organization's psychiatric care model, develop clinical workflows and protocols, and prepare for the launch of psychiatric services. As clinical operations begin, the role will gradually expand to include direct patient care through comprehensive psychiatric evaluations and ongoing medication management.
Working alongside clinical leadership, operations, and product, the Founding Lead Psychiatrist will help build an integrated model of care where psychiatry, therapy, care advocacy, and technology work seamlessly together to support children, youth, and families impacted by the child welfare system. This individual will serve as the organization's psychiatric thought partner, helping shape collaborative care pathways, clinical standards, and interdisciplinary workflows that improve access, quality, and outcomes for Medicaid-covered populations.
As the organization grows, the Founding Lead Psychiatrist will provide clinical leadership and supervision for psychiatric nurse practitioners and other psychiatric clinicians, fostering a collaborative, team-based culture while ensuring high standards for quality, safety, and evidence-based care.
This is a hands-on founding role for a psychiatrist who enjoys building as much as leading. The successful candidate is energized by early-stage innovation, comfortable rolling up their sleeves to solve operational and clinical challenges, and excited to help create-not just oversee-a new model of virtual behavioral healthcare. Key Responsibilities
Direct Patient Care
  • Conduct comprehensive psychiatric evaluations.
  • Diagnose behavioral health and psychiatric conditions.
  • Develop individualized treatment plans.
  • Provide medication management and ongoing psychiatric follow-up.
  • Collaborate closely with therapists, care coordinators, and primary care providers.
  • Maintain accurate, timely, and compliant medical documentation.
  • Participate in clinical case reviews and multidisciplinary care planning.
  • Partner with therapists on shared treatment planning to ensure coordinated care and appropriate integration of psychotherapy and medication management.

Clinical Program Development
  • Partner with clinical leadership, operations, and product to design and continuously refine the organization's psychiatric care model.
  • Develop evidence-based clinical workflows and prescribing protocols.
  • Assist in establishing quality metrics and clinical outcomes.
  • Recommend improvements to care delivery, documentation, and patient experience.
  • Support implementation of new clinical initiatives and service offerings.
  • Partner with product and operations to develop and optimize clinical technology, workflows, and virtual care experiences.

Clinical Leadership
  • Serve as the psychiatric subject matter expert and thought partner across the organization.
  • Partner with leadership across clinical, operations, and product to shape psychiatric strategy and service delivery.
  • Provide consultation on complex clinical cases.
  • Help establish policies, procedures, and clinical standards.
  • Support provider onboarding and psychiatric training initiatives.
  • Contribute as a founding clinical leader by helping translate innovative ideas into practical, scalable clinical processes.

Future Supervisory Responsibilities
As the psychiatric team expands, responsibilities will include:
  • Provide required physician supervision for psychiatric nurse practitioners where applicable.
  • Conduct regular case reviews and chart audits.
  • Mentor psychiatric clinicians to promote high-quality, evidence-based care.
  • Monitor clinical quality, documentation, and prescribing practices.
  • Support recruitment and onboarding of psychiatric providers.
  • Foster continuous professional development and clinical excellence.

Qualifications
Required
  • MD or DO with completion of an accredited psychiatry residency.
  • Board Certified or Board Eligible in Child & Adolescent Psychiatry.
  • Active medical license in Virginia (strongly preferred, or willingness and ability to obtain)
  • DEA registration and controlled substance prescribing authority.
  • Demonstrated experience diagnosing and treating children and adolescents in a virtual, outpatient behavioral health setting.
  • Ability to effectively engage children, adolescents, and their families in collaborative treatment planning.
  • Experience supervising or collaborating with psychiatric nurse practitioners.
  • Comfort working in an early-stage, fast-paced environment where clinical leaders contribute strategically while also helping build day-to-day clinical operations.

Preferred
  • Experience helping develop, launch, or scale virtual clinical programs, service lines, or models of care.
  • Experience caring for children and adolescents involved in the foster care and child welfare systems.
  • Experience collaborating with child welfare agencies, foster families, case managers, schools, and other community partners.
  • Thrives in the space of innovation, bringing sound clinical judgment, curiosity, and a collaborative approach to navigating evolving regulations, virtual care standards, and emerging models of pediatric psychiatric care.
  • Experience working with Medicaid populations and understanding the opportunities and challenges of Medicaid-funded behavioral healthcare.
  • Active medical license in MO, NM, LA, GA, NC, FL

Work Environment & Hours
  • 100% remote
  • Part-time role with flexible scheduling
  • Initial commitment of approximately 20 hours per month during the pre-launch phase, with responsibilities and time commitment expected to grow as psychiatric services launch (anticipated late Fall 2026).
  • Must have access to a private, quiet workspace with reliable high speed internet.
