We are seeking a dedicated and compassionate Licensed Physical Therapist to join our team at Unity Physical Therapy Clinic in Sanford, Florida. This full-time role is focused on providing high-quality care in a home care setting, where you will work directly with patients in their own environments to support their rehabilitation and mobility. The ideal candidate will hold a Bachelor's degree in Physical Therapy and have at least two years of clinical experience.

Work Environment

This position operates onsite, primarily in patients' homes, allowing for variability in your workday and the ability to form meaningful connections with patients and their families. Key Responsibilities
  • Conduct comprehensive patient evaluations, including detailed analysis of medical history and physical assessments to determine effective treatment plans tailored to individual needs.
  • Clearly explain evaluation findings to patients and their families, actively involving them in the goal-setting process to enhance compliance and motivation throughout the rehabilitation journey.
  • Implement treatment protocols guided by evidence-based practices, adjusting methods as necessary based on ongoing patient assessments and evolving healthcare standards.
  • Maintain open communication with other healthcare professionals involved in the patient's care, ensuring cooperative and holistic treatment approaches.
  • Document patient progress meticulously, creating accurate treatment records and discharge plans that reflect changes in patient status and future care recommendations.
  • Manage time effectively to meet productivity standards while also participating in community outreach and marketing initiatives to promote clinic services.
  • Contribute to achieving departmental objectives and regulatory compliance, engaging in initiatives that promote patient safety and service quality.
  • Engage in lifelong learning through continuing education opportunities to stay current on best practices in physical therapy, while also mentoring upcoming students as a clinical instructor.
  • Facilitate barrier removal for patient access to care, utilizing data to inform practices that improve client engagement and service effectiveness.

Qualifications
