• Conduct comprehensive evaluations by gathering pertinent patient history, performing specific assessments, and deriving insightful conclusions to formulate individualized treatment plans.
• Clearly articulate evaluation findings and treatment plans to both patients and their families, engaging them in goal-setting processes to ensure alignment and buy-in.
• Demonstrate expertise in implementing treatment protocols, adapting to the patient’s evolving health status while integrating best practices reflective of the latest healthcare innovations.
• Collaborate with interdisciplinary teams, recognizing when to document and communicate changes in a patient's condition that may necessitate adjustments in their plan of care.
• Oversee all necessary discharge and follow-up planning, ensuring accurate and timely documentation of care activities, including comprehensive progress notes and discharge evaluations that capture the patient's journey.
• Exhibit effective time management skills by maintaining a full patient schedule while meeting productivity targets and contributing to community outreach and marketing initiatives.
• Engage with departmental goals, actively participating in efforts to comply with accreditation requirements and governmental regulations.
• Pursue ongoing professional development and continuing education opportunities, promoting growth not only for yourself but also within the organization.
• Facilitate a positive patient experience by displaying adaptability and responsiveness to patient needs, working proactively to remove barriers to care.

You will primarily work with diverse patient groups, including elderly individuals recovering from surgery, people with chronic conditions, and others requiring rehabilitative support in their own homes. This role demands a high level of empathy and proficiency in tailoring treatment plans to meet the varied needs of each patient, establishing a therapeutic relationship built on trust and respect.

• A Bachelor's degree in Physical Therapy from an accredited institution.
• A High School Diploma or GED is also required.
• Licensure as mandated by state regulations for physical therapists.
• Current American Heart Association's BLS for Healthcare Providers CPR certification.
• No prior work experience is necessary, making this an excellent opportunity for recent graduates eager to start their careers.

While prior professional experience in physical therapy is not required, any clinical internships or volunteer work in rehabilitative settings will be advantageous. Familiarity with home health care and community rehabilitation practices will also enhance your ability to connect with our patients effectively.
