Reports To: Director of Rehabilitation (DOR) / Rehab Manager
Employment Type: Full-Time, Part-Time, Per Diem, or Contract
Position Summary
Teamwork Therapies is seeking a compassionate, motivated, and licensed Occupational Therapist (OT) to provide high-quality rehabilitation services in a Skilled Nursing Facility (SNF). The Occupational Therapist will evaluate residents, develop individualized treatment plans, provide evidence-based interventions, collaborate with interdisciplinary teams, and help residents achieve the highest possible level of independence, safety, and quality of life.
Essential Duties and Responsibilities
Patient Evaluation & Assessment
- ย Perform comprehensive occupational therapy evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions.ย
- ย Assess functional mobility, activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognition, upper extremity function, visual-perceptual skills, balance, coordination, and safety awareness.ย
- ย Identify adaptive equipment and environmental modification needs.ย
Treatment Planning
- ย Develop individualized treatment plans based on physician orders, resident goals, and interdisciplinary recommendations.ย
- ย Establish measurable short-term and long-term functional goals.ย
- ย Update treatment plans based on resident progress and changing medical status.ย
Direct Patient Care
Provide evidence-based occupational therapy interventions including:
- ย ADL retrainingย
- ย Upper extremity strengthening and ROMย
- ย Fine motor coordinationย
- ย Balance and postural controlย
- ย Functional mobility trainingย
- ย Wheelchair positioningย
- ย Transfer trainingย
- ย Splinting and orthotic managementย
- ย Cognitive rehabilitationย
- ย Visual-perceptual trainingย
- ย Energy conservation techniquesย
- ย Home safety educationย
- ย Fall prevention strategiesย
- ย Adaptive equipment trainingย
- ย Pain management techniquesย
Documentation
- ย Complete evaluations, progress notes, daily treatment notes, discharge summaries, and recertifications accurately and within required timelines.ย
- ย Maintain documentation that meets Medicare Part A, Medicare Part B, Managed Care, Medicaid, and payer requirements.ย
- ย Utilize the facility's electronic medical record (EMR) system.ย
Interdisciplinary Collaboration
- ย Participate in care plan meetings.ย
- ย Collaborate with physicians, nurses, speech-language pathologists, physical therapists, social workers, dietitians, and nursing staff.ย
- ย Communicate resident progress and discharge recommendations.ย
Patient & Family Education
- ย Educate residents and caregivers regarding:ย
- ย Home exercise programsย
- ย Adaptive equipmentย
- ย Fall preventionย
- ย Safe transfer techniquesย
- ย Joint protectionย
- ย Energy conservationย
- ย Home safety recommendationsย
Compliance
- ย Maintain compliance with:ย
- ย Medicare and Medicaid regulationsย
- ย CMS requirementsย
- ย State and federal regulationsย
- ย Facility policiesย
- ย HIPAA requirementsย
- ย Infection prevention proceduresย
Quality Improvement
- ย Participate in quality assurance and performance improvement initiatives.ย
- ย Assist with outcome tracking and clinical quality measures.ย
- ย Maintain productivity standards established by the facility.ย
Qualifications
Required
- ย Graduate of an accredited Occupational Therapy program.ย
- ย Current California Occupational Therapist license in good standing.ย
- ย Current CPR/BLS certification.ย
- ย Excellent written and verbal communication skills.ย
- ย Ability to work independently and collaboratively within an interdisciplinary team.ย
Preferred
- ย Minimum of one year of Skilled Nursing Facility or adult rehabilitation experience.ย
- ย Experience with Medicare Part A, Part B, Managed Care, and PDPM.ย
- ย Experience using Net Health, Rehab Optima, Casamba, PointClickCare, or similar EMR systems.ย
Knowledge & Skills
- ย Strong clinical reasoning and problem-solving abilities.ย
- ย Knowledge of geriatric rehabilitation principles.ย
- ย Familiarity with PDPM reimbursement guidelines.ย
- ย Excellent time management and organizational skills.ย
- ย Ability to prioritize caseloads efficiently.ย
- ย Strong interpersonal and customer service skills.ย
Physical Requirements
- ย Ability to lift, transfer, position, and assist residents safely.ย
- ย Frequent standing, walking, bending, reaching, kneeling, and lifting up to 50 pounds with or without reasonable accommodation.ย
- ย Ability to demonstrate therapeutic activities and exercises.ย
