Key Responsibilities
  • Conduct initial patient assessments to evaluate the physical and functional status of each client, gathering information essential for creating tailored rehabilitation programs.
  • Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans focused on achieving specific goals and fostering patient engagement.
  • Deliver hands-on therapeutic interventions, utilizing a variety of modalities and techniques, to promote patient recovery and mobility in accordance with the prescribed plan of care.
  • Instruct and educate patients, families, and caregivers on therapeutic exercises, the use of assistive devices, and strategies to enhance recovery at home.
  • Regularly assess patient progress, adjusting treatment plans as necessary to optimize outcomes and ensure functional independence.
  • Prioritize caseload management, determining daily staffing needs while coordinating patient assignments and team communications.
  • Document clinical findings and care provided in compliance with legal and ethical standards to maintain accurate patient records.
Patient Population / Client Focus

Qualifications
