• Conduct initial patient assessments to evaluate the physical and functional status of each client, gathering information essential for creating tailored rehabilitation programs.
• Collaborate with physicians and interdisciplinary teams to develop and implement individualized treatment plans focused on achieving specific goals and fostering patient engagement.
• Deliver hands-on therapeutic interventions, utilizing a variety of modalities and techniques, to promote patient recovery and mobility in accordance with the prescribed plan of care.
• Instruct and educate patients, families, and caregivers on therapeutic exercises, the use of assistive devices, and strategies to enhance recovery at home.
• Regularly assess patient progress, adjusting treatment plans as necessary to optimize outcomes and ensure functional independence.
• Prioritize caseload management, determining daily staffing needs while coordinating patient assignments and team communications.
• Document clinical findings and care provided in compliance with legal and ethical standards to maintain accurate patient records.

As a Physical Therapist at Peak Performance Therapies, your primary focus will be on treating adults and children recovering from surgeries, injuries, or chronic conditions requiring rehabilitative care. This includes working with patients post-operatively, individuals experiencing mobility challenges, and those in need of pain management. Your holistic approach to therapy will be critical in helping each patient achieve optimal recovery and functionality in their everyday life.

• A valid license to practice as a Physical Therapist in the state of Connecticut.
• A Doctor of Physical Therapy (DPT) degree from an accredited program.
• Basic Cardiac Life Support (BCLS) certification or the willingness to obtain it within 30 days of employment.
• No prior experience in the field is required; however, a passion for patient care and a strong willingness to learn and grow are essential.

• Experience or coursework related to home health physical therapy or familiarity with various rehabilitation modalities is preferred but not mandatory.
• Strong interpersonal communication skills for effective patient and family interaction.
• Effective time management abilities and organizational skills to handle a diverse caseload efficiently.
