Key Responsibilities
  • Evaluate patient functional abilities and disabilities including range of motion, strength, and coordination through standardized assessment and testing; establish functional baseline to gauge progress throughout treatment
  • Interpret and implement referrals; develop and execute individualized education and treatment plans including goal setting and estimating length of treatment
  • Provide direct patient care including individual and group physical therapy activities in accordance with the treatment plan; instruct patients in skills necessary for successful participation
  • Participate in interdisciplinary team discussions, rounds, and clinics; collaborate on patient care goals and progress
  • Maintain accurate, legible, and complete documentation of evaluations, treatment plans, treatments performed, and patient progress in the electronic health record per compliance timelines
  • Provide family and community education; serve as a resource and deliver in-services in areas of expertise within the rehabilitation system
Required Qualifications
  • Valid Michigan Physical Therapist license required
  • BLS certification through the American Heart Association (AHA) required
  • Minimum of 1 year of physical therapy experience required
  • Home health experience required
  • Valid Michigan driver's license, daily access to a personal vehicle for business use, and proof of auto insurance required
  • Supervisor-level references required
Preferred Experience / Skills
  • Experience with splint design and fabrication or assistive device creation
  • Experience with electronic health record documentation systems
  • Background in program development or department process improvement initiatives
