Job description

Clinical Liaison

The Clinical Liaison is responsible for creating and implementing an effective marketing strategy, building strong relationships in the community, increasing patient census, and fostering partnerships with physicians, clinics, hospitals, Assisted Living Facilities, Hospice providers, and Home Care companies. This position also gathers necessary information to support the admission process and assists with transitioning patients from home, hospital, or other facilities.

Clinical Liaison Responsibilities:
  • Build and maintain relationships with acute‑care hospitals, healthcare systems, discharge planners, case managers, rehabilitation centers, and assisted living facilities

  • Collaborate with physicians and hospital discharge planners to arrange smooth resident transitions

  • Maintain ongoing relationships with physicians, social workers, and referral sources

  • Assist in developing and implementing marketing strategies in partnership with regional marketing teams and facility leadership

  • Develop marketing materials including brochures, flyers, ads, and campaigns

  • Maintain responsibility for patient referrals until admission is completed

  • Demonstrate knowledge of inpatient rehabilitation and LTACH admission criteria

  • Conduct on‑site pre‑admission assessments when appropriate

  • Utilize fast‑track processes to support referral conversions

  • Participate in community events and activities that support company goals

  • Provide leadership in strategic planning and identify opportunities for improved services

Clinical Liaison Qualifications:
  • LPN or RN preferred

  • Minimum of two years’ experience in the field of licensure, ideally with rehabilitation, LTACH, and/or marketing experience

  • Proven experience in marketing, preferably in senior living or skilled nursing

  • Strong marketing skills with the ability to engage customers and the community tactfully

Travel
  • Approximately 80% local travel is expected for this position
