Home Health RN Case Manager Jeffersonville, IN $56.14/hour + $0.46/mile Full-Time | Mondayโ€“Friday, 8:00 AM โ€“ 5:00 PM Contract-to-Hire About the Role A-Line Staffing is seeking a compassionate and organized Home Health RN Case Manager to provide high-quality care coordination for patients in Jeffersonville and surrounding areas. This role is ideal for an RN who excels in clinical assessments, care planning, and patient advocacy, with a focus on improving outcomes in a home health setting. Coverage Area Harrison County Crawford County Orange County Partial Floyd County Washington County Additional coverage may be required for PTO coverage, call-ins, and admissions.

Key Responsibilities Perform comprehensive assessments of high-risk patients Develop and implement individualized care plans Collaborate with physicians, social workers, discharge planners, and interdisciplinary teams Coordinate care transitions and ensure appropriate levels of care Arrange non-medical support services, including transportation and housing Engage specialty resources to optimize patient outcomes Maintain accurate clinical, functional, and fiscal documentation Identify opportunities for health promotion and illness prevention Intervene to prevent adverse patient events Serve as a patient advocate while maintaining privacy and confidentiality Schedule & Workload Full-Time, Mondayโ€“Friday, 8:00 AM โ€“ 5:00 PM Weekend on-call rotation required Productivity expectation: 30 points weekly Qualifications Active, unrestricted RN license in Indiana (Compact license accepted) Associateโ€™s or Bachelorโ€™s Degree in Nursing (or related field) Strong clinical judgment and analytical skills Case Management certification preferred Preferred experience: Wound care, IV care, Drain care, G-tube care, Patient education Homecare Homebase (HCHB) experience preferred Why Apply?
