We are currently seeking a Transition Coordinator to join our Transitional Services program. As a Transition Coordinator, you'll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
  • Act as point-of-contact to assist the individual to live in his/her home and/or community.
  • Assist individuals served with linkages to service providers and community resources as needed.
  • Identify barriers to compliance with individual's healthcare regime, appointments and screenings.
  • Work with individuals to create solutions to identified barriers and empower individuals to manage physician, and/or other key medical professionals, contacts, appointments and/or screenings.
  • Communicate and collaborate with the individual's healthcare team, including, but not limited to, the facility Social Worker and Waiver Case Manager.
  • Adheres to the CareStar Rule in performance of job responsibilities.
  • Understands and complies with CareStar Policies and Procedures.
  • Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
  • Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.

Minimum Qualifications
  • At least 18 years of age or older.
  • High School Diploma or higher education level.
  • Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting.
  • Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities.
