Position Summary:
The Director of Clinical Transition is responsible for implementing and monitoring current policies and procedures for Clinical Services and ensuring all assigned communities stay in compliance with all state, federal, and government agencies. You will conduct compliance surveys at assigned communities and be present for, and participate in as necessary, any state and federal survey activity. Regular site visits to provide supervision, clinical expertise, and to monitor the overall operation of clinical services will be required. You will also provide in-service training and orientation to facility staff regarding survey process, risk management, community compliance, and performance improvement, review and investigate resident complaints as directed, and develop Plans of Corrective Action as needed with the facility Administrator and Director of Nursing.
Qualifications/Education:
  • Must possess an active, unencumbered RN license.
  • Must have at least two (2) years' experience as a Director of Nursing (DON) or Regional Nurse.
  • Must be willing to travel to facilities in multiple states based on the location of acquisition facilities.
