What will you be doing:

Responsible for processing patient referrals and maintaining client information in database. Provides clerical and administrative support to DME Customer Service in day-to-day care management activities. Provides telephonic support for incoming and outgoing calls from patients, providers, Transition Coach and other team members related to the Transition Care program. Responsible for expanding, updating and maintaining community resource database.

What will you come with:

  • 1-2 years of customer service support experience with the healthcare industry, Home Health Services preferred.
  • Effective verbal and written communication skills.
  • Ability to use various computer programs and applications, IE EMR/EHR
  • Ability to self-motivate and work independently.
  • Customer focus and adaptability to different personality types
  • Ability to multi-task, set priorities and manage time effectively.
  • Familiar with Patient Referrals
