Job Overview:

The Specialist functions semi-independently, under the direction of the Transfer Center administration as part of the integrated healthcare team. The Transfer Center Specialist will work to enter Epic referrals into About, preadmit & send insurance verification to Patient Access, confirm all necessary components of referral are present and complete, document clear follow up for clinicians, answer phones, at time of patient acceptance – confirms appropriate bed requests are placed, coordinates transport with PTC or otherwise appropriate entity, and ensures patient’s timely arrival to inpatient unit. Knowledge and necessary skills will be utilized to effectively meet the needs of transfer process. Must possess ability to establish and maintain effective working relationships with the public.  Requires the ability to effectively plan; must be able to follow directions and to perform work according to department standards when no directions are given. Requires enthusiasm, initiative, and the ability to work independently/cooperatively with other personnel. Requires the ability to function effectively in a fast-paced environment with multiple demands/deadlines.

Qualifications:

Required:

  • High school diploma/GED
  • 2-3 years of experience in a healthcare office or hospital operations setting, and/or educational background

Preferred:

  • Expert level of Microsoft Office 
  • Ability to accurately type at least 40 wpm

Scheduling Requirements:

  • Full-Time, Day 

Essential Functions:

  • Serves as the point of contact for referrals and obtains necessary information to initiate and  facilitate the patient’s transfer into RWJBH Healthcare hospitals under administration and charge supervision
  • Proficient in all Transfer Center workflows and protocols
  • Works directly with staff at each client facility to ensure timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate destination plan of care at receiving facility.  Receiving, documenting, and communicating time-sensitive clinical data from a multitude of sources.
  • Demonstrates the ability to triage and prioritize patient information under the direction of the clinician 
  • Coordinates transports and pick up times
  • Proficient in Transfer Center database platform, EMR, telephony systems, and Office systems as required to support operations, ensuring accurate, timely data entry, protection of PHI
  • Demonstrates knowledge of regulatory components to include but not limited to, Ethics and Compliance, quality initiatives, and HIPAA; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives
  • Effectively supports system, facility and department goals and objectives, mission, vision, and values through appropriate use of interpersonal skills and job performance. Support Transfer Center and RWJBH hospital's growth strategies
  • Demonstrates well-defined organizational and time-management skills. Identifies, analyzes, organizes, and solves problems and issues in a timely, effective manner and collaborates with others to make sound, timely decisions
  • Demonstrates commitment to and compliance with company’s policies and procedures.  Adheres to safety and infection control and prevention policies, procedures and guidelines
