PRN Home Health & Therapy is seeking a dedicated Occupational Therapist to join our growing team. This full-time position provides the freedom to manage your own schedule while making a lasting impact on patients throughout Appleton, Oshkosh, and the surrounding communities.


In this role, you'll primarily provide care in a home health setting, with occasional visits to skilled nursing facilities. Whether you're an experienced clinician or a new graduate eager to begin a rewarding career in home health, this position offers an excellent opportunity to grow and succeed.


Key Responsibilities

  • Work independently while maintaining strong communication with the clinical team.
  • Review and respond to patient referrals in a timely manner.
  • Collaborate with patients, caregivers, and interdisciplinary team members to develop individualized care plans.
  • Create and implement treatment plans with measurable, patient-centered functional goals.
  • Educate patients and caregivers to support successful outcomes and maximize independence.
  • Provide guidance, direction, and supervision to Certified Occupational Therapy Assistants (COTAs).
  • Assess and recommend durable medical equipment (DME), including wheelchairs, adaptive equipment, and other assistive devices.
  • Partner with healthcare professionals to ensure coordinated, high-quality patient care.
  • Perform additional duties as assigned.
