Suburban Home Health is a jointly owned venture between several Suburban Health Organization hospital systems here in central Indiana! We work as an extension of the hospital consortiums you trust in your community, with the goal of improving care across the continuum for our homebound patients. Job Description
The Speech Therapist evaluates speech language skills for adult and/or geriatric patients as related to medical, social and psychological factors in the home setting.
  • Performs appropriate diagnostic, neurological and functional skills tests and establishes a plan of treatment.
  • Plans and conducts rehabilitative treatment programs including vocal exercises to restore patient's communicative abilities.
  • Instructs and assists patients in the performance of communication, cognitive and dysphagia treatment to maximize therapeutic outcomes.
  • Monitors patient progress in therapy and collaborates with the interdisciplinary team to ensure patient outcomes are achieved.
  • Documents the initial evaluation and each day of treatment in the patient record in a timely manner and completes summary of patient's progress at discharge.
  • Provides education to the patient and caregiver regarding methods of care provided.

Requirements
Basic Qualifications
  • Must be a graduate of an accredited school of speech pathology and holder of a current permanent or temporary license from the Indiana Medical Licensing Board.
  • Must possess Certificate of Clinical Competence in speech pathology
  • Must possess exceptional verbal and written communication skills.
  • Ability to prioritize work, utilizing fundamental time management skills.
  • Must possess valid Driver's License and automobile insurance.
