TCM Child Placement Specialist

Pay scale: Starting at $17.31/hr.

Summary

Under direct and indirect supervision, provides Tailored Care Management (TCM) providing comprehensive support for children and adults assigned to Daymark for TCM services who require out-of-home placement (e.g., group home, PRTF). Possesses strong organization and communication skills to assist Placement Specialist QP with obtaining necessary documentation for placement referrals, provides monitoring/follow up during placement and after discharge to ensure appropriate services are in place.

Essential Duties and Responsibilities

  • Provides comprehensive care management for individuals in the Daymark TCM program who require out-of-home placement. Collaborates closely with individuals, legal guardians, and/or natural supports to gather and provide all essential information for the placement process.
  • Performs monitoring/follow up with individuals, legal guardians, natural supports, and placement child, guardian. Attends Child and Family Team (CFT) meetings and other treatment team meetings with placement.
  • Assists with 90-day transition plan and monitors engagement in step-down services.
  • Maintains thorough documentation of all activities in accordance with established documentation and billing guidelines.
  • Routinely requests records from other entities to gather necessary documentation needed for placement referrals. File, release and copy protected healthcare information. Answers phone calls, transfers to the appropriate parties, is courteous to the public.
  • Assists QP with other care coordination duties for placement and discharge
  • All other duties as assigned by supervisor

Responsibilities of the TCM Child Placement Specialist include, but are not limited to, the following:

  • Reviews all information in client's record and reviews with QP and supervisor to discuss case and immediate placement needs.
  • Makes referrals to emergent placements when necessary (i.e. FBC or inpatient hospital).
  • Works closely with other TCM staff and if needed other Daymark departments to ensure all necessary documentation needed for placement are obtained (i.e. CCA, ROIs, etc)
  • Requests and gathers all other documents needed to make placement referrals (copies of birth certificate, immunization record, medical records, school records, and any other information requested by placement).
  • Tracks and document all placement efforts for client.
  • Communicates regularly with client, legally responsible person and all other care team members to provide updates on placements efforts and progress.
  • Once placement is obtained, coordinates with QP to ensure placement is finalized. This includes scheduling intake and coordinating transportation to placement for client. Also, if necessary, works with care team to coordinate 30 day supply of medication (requirement for many residential placements)
  • Works closely with the client, legally responsible person, and placement provider agencies to ensure services in the Care Management Plan are adequate and effective.
  • Documents monitoring and the actions taken/planned as a result of the monitoring in the consumer's record.
  • In collaboration with placement, assist with discharge planning to ensure step-down services are in place and other needs addressed to include transportation and medication management.

Special Attributes: Must possess a good attitude, work well with others, have a strong commitment to patient care. Minimal supervision regarding use of time, able to prioritize work assignments. Ability to communicate effectively with professions and clients/families. Have knowledge of Mental Health, Social Work or Health Care Fields.

Education and/or Experience:

Minimum of a high school diploma or equivalent.

AND meet one of the following criteria:

  • Certified Peer Support Specialists;
  • Community health workers (CHW), defined as individuals who have completed the NC Community Health Worker Standardized Core Competency Training (NC CHW SCCT);
  • Individuals who served as Community Navigators prior to the implementation of Tailored Plans; Parents or guardians of an individual with an I/DD or a TBI or a behavioral health condition (parent/guardian cannot serve as an extender for their own family member);
  • A person with lived experience with an I/DD or a TBI or a behavioral health condition
  • Or 2 years of paid care management type experience with at least 1 year paid experience at any time with population served.

TCM trainings will be required to completed as assigned
