Key Responsibilities

  • Participate actively in case management and care coordination, contributing to treatment team meetings and utilization reviews.
  • Implement crisis intervention strategies to effectively address clients in immediate need of support.
  • Research and provide referrals for community resources, such as housing, healthcare, and social services to promote client well-being.
Patient Population / Client Focus

Qualifications
