As a Licensed Clinical Social Worker (LCSW) at Sutter Health Counseling, you will play a critical role in providing comprehensive clinical social work services remotely to our clients in State College, Pennsylvania. • Conduct clinical social work evaluations to assess clients' psychological and emotional well-being, and develop tailored treatment plans.
• Monitor clients' medication side effects in collaboration with psychiatrists, ensuring safety and efficacy in treatment.
• Develop aftercare plans that enable clients to successfully transition from intensive care settings to independent living.
• Document and maintain case records in compliance with company policies, including biopsychosocial assessments and progress notes.
• Participate actively in case management and care coordination, contributing to treatment team meetings and utilization reviews.
• Implement crisis intervention strategies to effectively address clients in immediate need of support.
• Research and provide referrals for community resources, such as housing, healthcare, and social services to promote client well-being.

This role primarily serves individuals dealing with complex health challenges, particularly those at end-of-life stages, as well as their families. You will assist clients in navigating emotional distress, bereavement, and the psychological impact of chronic health conditions. Your expertise will foster resilience and coping strategies in a supportive environment aimed at enhancing overall quality of life.

• A Master's Degree in Social Work from an accredited institution is required.
• Valid and active Licensed Clinical Social Worker (LCSW) designation.
• A minimum of 2 years’ experience in a behavioral health setting, with a history of effective case management.
• Strong competencies in conducting psychosocial assessments and crisis interventions.
• Excellent communication and interpersonal skills, with the ability to manage conflicts sensitively and effectively.

• Prior clinical experience specifically in hospice or palliative care settings.
• Familiarity with electronic health record (EHR) systems for managing client documentation and communication.
• Experience in establishing community partnerships to facilitate client access to necessary resources.

This position offers a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience and qualifications. In addition to competitive compensation, we provide a robust benefits package, including:
