Key Responsibilities- Conduct thorough assessments for new clients experiencing major mental illnesses and significant emotional disorders.
- Develop and implement individualized care plans that address the unique needs of each patient and their families.
- Provide individual, group, and family therapy sessions, ensuring that treatment services align with clients' presenting problems and requests.
- Facilitate timely interventions during crisis situations and provide support to clients and families navigating complex emotions associated with end-of-life care.
- Collaborate with interdisciplinary teams, including physicians, nurses, and external resources, to improve holistic care delivery.
- Regularly review and monitor clients throughout their treatment journey, ensuring that their goals and needs are met effectively.
- Submit timely reports and maintain accurate client records in accordance with organizational guidelines.
- Participate in quality improvement initiatives and contribute to committees aimed at enhancing agency service delivery.
This position focuses on a diverse clientele within the hospice setting, primarily working with patients in their final stages of life and their families. These clients may experience a range of emotions including grief, anxiety, depression, and existential distress. The LCSW will be expected to provide empathetic support that honors the unique journey of each patient, facilitating meaningful discussions around care preferences, family involvement, and coping strategies.
Qualifications- A Bachelor’s degree in Social Work or a related field; a Master's degree is preferred.
- Valid state licensure as a Clinical Social Worker (LCSW) in Texas.
- Minimum of 2 years of direct clinical experience, preferably in hospice, palliative care, or similar settings.
- Strong understanding of mental health disorders and crisis intervention strategies, particularly in the context of end-of-life care.
- Ability to engage effectively with clients from diverse backgrounds, demonstrating cultural competence and sensitivity.

The compensation for this role ranges from USD 70,000 to 85,000 annually, depending on experience and qualifications.
