About the Position

Katy Integrative Gastroenterology is seeking an energetic, compassionate, and highly organized Health & Wellness Coach to support our registered dietitian and growing nutrition and metabolic wellness programs.

The Health & Wellness Coach will help patients turn clinical and nutrition recommendations into sustainable daily habits. This person will provide encouragement, education, accountability, and ongoing support while coordinating closely with our dietitian, physicians, and advanced practice providers.

This is an excellent opportunity for someone who is passionate about nutrition, digestive health, weight management, metabolic wellness, and behavior change.

Key Responsibilities

• Provide individual health and wellness coaching based on care plans established by the dietitian or medical provider
• Help patients set realistic goals related to nutrition, physical activity, sleep, stress management, and other lifestyle habits
• Conduct scheduled coaching and accountability sessions in person, by telephone, or virtually
• Reinforce nutrition and lifestyle recommendations provided by the registered dietitian and clinical team
• Use motivational interviewing and behavior-change techniques to help patients overcome barriers
• Monitor patient progress, celebrate successes, and help patients remain engaged when progress is challenging
• Review patient food logs, activity tracking, and wellness questionnaires within the established care plan
• Document coaching sessions and patient communications accurately in the electronic medical record
• Identify concerns requiring evaluation by the dietitian or medical provider and communicate them promptly
• Coordinate nutrition and wellness appointments, follow-up visits, and program enrollment
• Contact referred patients, explain available programs, and assist them with scheduling
• Follow up with patients who miss appointments or become disengaged from their care plans
• Help patients use approved educational materials, tracking tools, and digital health resources
• Facilitate group coaching sessions, workshops, challenges, and patient education programs
• Assist with community outreach and wellness-related events
• Track program outcomes, patient engagement, retention, and completion of follow-up visits
• Maintain patient privacy and comply with HIPAA and all practice policies


Scope of the Position

The Health & Wellness Coach supports patients in carrying out care plans established by licensed healthcare professionals. The coach does not independently diagnose medical conditions, prescribe medications or supplements, develop medical nutrition therapy plans, or replace the registered dietitian or medical provider.

• Familiarity with motivational interviewing and evidence-based behavior-change strategies 

• Strong interpersonal skills and the ability to build trust with patients 

• Positive, encouraging, and nonjudgmental communication style 

• Excellent organization, follow-through, and documentation skills 

• Comfortable working with electronic medical records and basic spreadsheets 

• Bilingual English/Spanish is a plus
