Work Environment

This position is fully remote, allowing for flexibility while maintaining a strong commitment to client care. Key Responsibilities
  • Conduct comprehensive clinical evaluations to assess clients’ psychosocial needs and develop individualized treatment plans that promote successful transitions post-discharge.
  • Provide counseling sessions, utilizing evidence-based therapeutic techniques tailored to the unique circumstances of each client while ensuring ongoing monitoring of medication and potential side effects under the supervision of a Psychiatrist.
  • Document all interactions and care decisions diligently, including biopsychosocial assessments, treatment goals, progress notes, and discharge planning in compliance with company policies and state regulations.
  • Participate actively in multidisciplinary treatment team meetings and case management discussions, advocating for clients’ needs and ensuring continuity in care.
  • Research, refer, and advocate for community resources that can improve clients' well-being, including access to food assistance, childcare, health care services, and other essential resources.
Patient Population / Client Focus

This role focuses on clients within a hospice setting, many of whom are experiencing end-of-life transitions. As a Clinical Social Worker, you will support patients and their families, addressing emotional, psychological, and social challenges associated with serious illness. You will also work with diverse populations, requiring cultural competency and sensitivity to varying backgrounds and needs.

Qualifications
  • A Master’s Degree in Social Work (MSW) from an accredited institution is mandatory.
  • Current and unrestricted Licensed Clinical Social Worker (LCSW) certification is required.
  • A minimum of 2 years of experience in a behavioral health setting, particularly with vulnerable populations facing significant health challenges.
  • Strong skills in conflict management and problem resolution, enabling effective support for clients in crisis situations.
  • Demonstrated proficiency in conducting comprehensive psychosocial assessments and case management evaluations.
Preferred Experience
  • Experience in hospice care or palliative services is highly desirable.
  • Familiarity with electronic health records and telehealth frameworks.
  • Evidence of ongoing professional development or certifications in relevant therapeutic modalities.
