We are currently hiring a Community Health Worker – Problem Gambling based in Worcester. The Community Health Worker (CHW) – Problem Gambling is a member of the Problem Gambling Program which works alongside our Community Health Worker team. This role will work under the supervision and guidance of the designated CHW Supervisor on all phases of the problem gambling program development. This will include but will not be limited to planning phase to determine and assess the impact of problem gambling on communities in our service areas, development of assessment tools, implementation of all elements of the program and constant evaluation and feedback on the program. The role will work with community partners, patients of Kennedy Community Health and community members. This position requires flexibility and may vary from day- to-day to meet members where they are this will include many off-site meetings and events as the program develops. Outreach methods may vary based on the needs of the organization and may include telephone or in person in a variety of potential settings such as but not limited to, the community, home facility or health center.

Essential Functions:
  • Assists with development and implementation of an assessment tool for the designated service areas by working with community and program partners.
  • Identifies and maintains a listing of community resources, agencies and partners as referral sources for effected persons and their families.
  • Refers individuals with problem gambling to identified support and services and assist with access to those programs.
  • Conducts community and individual education sessions and distributes information sheets with resources available for individuals and families.
  • Attends public and strategic events in identified high risk areas.
  • Develops and implements outreach plans in collaboration with team colleagues, based on individual, family and community needs, strengths and resources.
  • Shares community assessment results with colleagues and community partners to inform planning and health improvement efforts.
  • Addresses language and cultural barriers to care.
  • Coaches and guides member/representative to meet both personal and clinical goals.
  • Completes Social Determinants of Health (SDOH) screenings and addresses identified insecurities/needs by providing information on community resources.
  • Works a fluctuating work schedule including day, evenings, and occasionally a Saturday shift (9am-1pm).
Education and Experience:
  • Proof of completion of the CHW core competency training
  • 2 years of experience working as a Community Health Worker
  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Depending on the nature of the position, bilingual in English and another critical language for the health center may be required.
