Additional Information About the Role
Parkland Health Center has a new, grant-funded position created through the Rural Health Transformation Grant and represents the first step in advancing regional healthcare transformation efforts. The role will support one of the state's hubs and help lead initiatives across three regional hubs and counties, working with stakeholders to identify priorities and drive key community health improvement efforts.
The position will be responsible for facilitating and coordinating transformation work, partnering with community organizations, behavioral health providers, ambulance districts, public health agencies, and other healthcare stakeholders.
Ideal candidates will demonstrate professionalism, excellent communication skills, community-focused leadership, and experience or interest in public health and community collaboration.
Overview
As part of BJC HealthCare, Parkland Health Center offers a unique combination of world-class medicine along with the deep local roots and rich history of its three predecessors - Bonne Terre Hospital (opened in 1911), Mineral Area Regional Medical Center (1952), and Farmington Community Hospital (1969). Parkland Health Center has two locations in St. Francois County, Missouri - a full-service acute care hospital in Farmington as well as a rural emergency hospital in Bonne Terre. Parkland Health Center provides quality primary and secondary level inpatient and outpatient services to the people of St. Francois County and surrounding areas in southeastern Missouri.
In addition, Parkland's Farmington campus offers a full continuum of inpatient and medical-surgical and intensive care, obstetrics and emergency services, along with a wide variety of outpatient services including oncology services, an infusion center, a wound care clinic, and the Huckstep Heart and Lung Center. Bonne Terre is a Rural Emergency Hospital providing emergency services and outpatient services including medical observation, lab, radiology, and sleep study.
Preferred Qualifications
Role Purpose
Develops and maintains a network of community resource centers, including development and implementation of curriculum, services and resource development for all locations. Responsible for convening strategic partners and developing collaborations within the community to improve our program relations and expand our service locations. Conducts ongoing needs assessment of surrounding community/neighborhoods to determine the most relevant topics for health education in the community resource centers. Responsible for assisting in planning, coordinating and implementing family connection meetings in the community. Facilitates recruitment and retention of participants in all program offerings.
Responsibilities
  • Develop strategic community partnerships at agencies in the metro-area for our team members to have identified drop-in office space and meeting space in the community
  • Plan, coordinate and implement education and access to resources at community resource centers.
  • Create and implement recruitment strategies for program participants.
  • Create and implement retention strategies for program participants.
  • Conduct regular resource needs assessments within the communities we serve.
