We are currently hiring a Community Health Worker based in Milford MA.  The Community Health Worker (CHW) is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home.  The CHW facilitates access to care and services by providing education and advocacy to assist patients in navigating the health care and social service systems.

Essential Functions:

  • Conduct psycho-social assessments with patients referred for Community Health Worker interventions in accordance with Health Center policies, procedures and protocols.
  • Provide health system education and advocacy to assist patients in navigating the health care system.
  • Provide basic education, screening, brief intervention and/or facilitate referrals to other health center or community-based resources in accordance with applicable health center protocols.

Job Requirements:

  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Successful completion of the CHW core competency training within 2 years of employment.
  • BLS Certification, can be obtained through the Health Center
