Position Summary: Weโ€™re looking for a Personal Outcomes Coach whoโ€™s ready to spread kindness, joy, and fun while supporting participants in our Independent Supported Living Assistance (ISLA) Program. In this flexible and rewarding role, youโ€™ll provide and oversee ongoing training regarding daily living skills, ensuring that people have every opportunity to achieve their personal outcomes. Youโ€™ll be the source of positivity, making sure every day is filled with meaningful moments, and yesโ€”plenty of fun too!


What Youโ€™ll Do:

ยท Empower with Kindness: Provide continuous support and training to help people achieve their personal goals as outlined in their Individualized Support Plans (ISPs), all while bringing a sense of kindness and encouragement.

ยท Lead with Joy and Fun: Ensure that all guidelines are met or exceeded and do it with a joyful spirit that makes the process enjoyable for everyone involved.

ยท Celebrate Success: Keep detailed progress notes daily, ensuring compliance with funding guidelines while celebrating every win, big or small, and making sure to have fun along the way.

ยท Be a Source of Positivity: Attend meetings, maintain excellent documentation, and communicate consistently with individuals, guardians, and service providersโ€”all with a smile, a positive attitude, and a sense of fun.

ยท Ensure Safety with Care: Assist with developing and reviewing individualized safety plans and maintaining safety supplies, always prioritizing the well-being and comfort of the individuals while keeping the environment lively and engaging.

ยท Radiate Sunnyhill Spirit: Represent our organization with professionalism, kindness, and a joyful, fun-loving attitude in everything you do.

Qualifications:

  • A passion for helping others and a commitment to empowering individuals with IDD.
  • Strong organizational skills and attention to detail.
  • Excellent communication skills, both written and verbal.
  • A positive, can-do attitude and the ability to work both independently and as part of a team.
  • Previous experience in a similar role is a plus, but weโ€™re also excited to train motivated individuals who are new to the field!
