Job description


The behavioral healthcare provider is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant. The behavioral healthcare provider is responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload with the patient's medical provider and consultant psychiatrist. The behavioral healthcare provider meets with patients independently as a professional therapist and serves as an advocate for holistic patient care.
Responsibilities
  1. Support the mental and physical health care of patients on an assigned patient caseload.
    1. Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow-up care.
    2. Provide patient education about common mental health and substance abuse disorders and the available treatment options.
    3. Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
  2. Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
    1. Document patient progress and treatment recommendations in EHR so that the information can be shared with the care team.
    2. Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant which includes more intensive specialized mental health care outside of the primary care clinic.

Qualifications
  • 2 years of experience as a Licensed Clinical Social Worker with knowledge and experience in mental health therapeutic processes and counseling techniques.
  • Master of Social Work
  • Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
  • Licensed Clinical Social Worker (LCSW)
