JOB TITLE: Student Assistance Counselor- School Based
SUMMARY:
Provide professional case management services as needed including assessment, treatment planning addressing social determinants of health, closing HEDIS gaps and proving care coordination with internal and external medical providers.
EDUCATION AND EXPERIENCE:
Education: Bachelor's degree in behavioral health-related field required.
Licensure: N/A Experience: Experience in the field of behavioral health. Experience in school based behavioral health services preferred.
Knowledge/Skills:
Verbal/written communication skills. Ability to safely operate motor vehicle.
Ability to effectively address individual, small and large groups. Creative problem solving
Ability to coordinate multiple tasks, and build rapport with youth/young adults
Knowledge of the focus population
Self-directed
Must possess the ability and desire to learn new skills to include cultural competency, procedures and rights, addressing the needs of transition age youth, and effective self-care and independent living strategies Committed to the principals of a school based behavioral health position.
EQUIPMENT: Computer, general office equipment, motor vehicle and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES:
1. Demonstrate knowledge of requirements for clients needing behavioral health services in a school setting, including any specific contract or grant expectations associated with this position.
2. Obtain necessary training in trauma or other EBT interventions necessary to best perform their job duties.
3. Provide outreach education, advocacy, promotion, referral, training, and assistance for families, facility, and students referred for behavioral health interventions.
4. Identifies the behavioral health needs of students as referred.
5. Assists families with securing services and makes referrals for additional behavioral health services as needed.
6. Utilizes the behavioral / mental health screening tools in coordination with the school-wide system of social-emotional supports for early identification of students at risk.
7. Provide written documentation to complete all necessary chart work in accordance with policy on admissions, treatment, and discharges in a timely manner, and maintains any required documentation regarding students served and services provided.
8. Maintain client confidentiality according to Frontier Health policies.
9. Remain current in knowledge of community resources and how to access those resources.
10. Keep supervisors apprised of all critical or emergency situations.
11. Respond to emergency/crisis or walk-in situations calmly, rationally and in accordance with center policy, school district policy, and procedures.
12. Be available to consumers assigned as open clients 24 hours a day for crisis assistance. After hours, the agency's after-hours system may be utilized; however, the on-call staff or school administration may contact the Student Assistance Counselor as appropriate.
13. Must be able to safely operate a motor vehicle to ensure the delivery of community-based services.
14. Attend and participate in assigned and scheduled staff meetings, IEP meetings, school administration meetings as requested, in-services, committees, conferences, assigned special school events, and individual program planning, and supervision as needed.
15. Disseminate educational information through presentations to school personnel and consultation for school staff as requested.
16. Participate in in-service training and orientation and provide consultation and education services to the community outside of the school itself and as requested by supervisor/administration.
17. Develop and maintain collaborative and effective working relationship with students, families, school facility and administration and other invested community stakeholders.
18. All other duties as assigned.
