Shift: Day ShiftPosition Summary:

We are seeking a compassionate and dedicated Licensed Social Worker (LSW) to provide psychosocial support, counseling, and care coordination services to patients and their families. The ideal candidate will assess patients' social, emotional, and environmental needs and collaborate with interdisciplinary healthcare teams to develop and implement effective care plans.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments of patients and families.
  • Develop, implement, and monitor individualized care and discharge plans.
  • Provide counseling, crisis intervention, and emotional support services.
  • Coordinate community resources and referrals to support patient needs.
  • Advocate for patients and families to ensure access to appropriate services.
  • Collaborate with physicians, nurses, case managers, and other healthcare professionals.
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements.
  • Assist patients with transition planning, including discharge and post-discharge support.
