Job Summary:

Provides expert client care by facilitating highly specialized psychosocial assessments, diagnostic impressions, and treatments for individuals, couples, families, and groups, while collaborating with senior leaders. Works with interdisciplinary teams to develop and implement individualized care plans, addressing complex social service needs, and communicating treatment guidance empathetically to patients. Drives social work service plans and expertly handles referrals and information sharing. Manages a caseload of patients with medium to high psychosocial acuity, collaborating with clinicians to ensure quality care and guiding improvements in care coordination, while developing case management practices. Ensures compliance by staying current with regulations and maintaining precise documentation. Contributes to program development by evaluating service impact, seeking new knowledge, and implementing improvements to enhance social work programs and protocols.

  • Provides effective client care by serving as an expert in facilitating highly specialized or advanced psychosocial assessment, diagnostic impressions, treatment of individuals/couples/families/groups, and prevention of psychosocial dysfunction/disability/impairment including emotional/mental/behavioral disorders, with senior leaders; collaborating with interdisciplinary teams when conducting psychosocial assessment, including functional status and financial resources to develop and implement an individualized care plan, determine interventions needed, and discuss social service needs; communicating and providing guidance to patients in an empathetic and supportive way to share results, discuss social work and treatment plans, provide resources, and answer complex, clinically-based questions with empathy and support; driving and implementing social work service plans and proactively seeking and applying feedback; and referring members, patients, and staff to the appropriate services and proactively sharing information with providers.
  • Coordinates the care of a caseload of patients with medium to high psychosocial acuity by collaborating with treating clinician to review and ensure quality of patient treatment plans; advising, guiding, and/or coordinating services to improve care coordination based on quality principles; using advanced knowledge to recommend patient referrals for moderately complex cases; developing and maintaining case management policies and procedures to ensure optimal and appropriate member utilization and engagement of services; and solving problems concerning patient treatment plans and follow-up appointment documentation.
  • Enables compliant services by maintaining up-to-date awareness of organizational, state, and federal guidelines as well as regulatory policies, standards, and procedures, to ensure compliance; and proactively submitting written and electronic documentation, records, and reports (e.g., charting) according to department, federal, and state regulations.
  • Contributes to program development and management by evaluating the quality and effectiveness of social work programs and identifying opportunities for new program development that consider the impact of services; and proactively seeking updates and new knowledge on industry best practices and new treatment options to maintain systems and processes for outcome management as well as recommending and implementing areas of improvement for social work programs or protocols.

Minimum Qualifications:

  • Minimum one (1) year of experience in a leadership role with or without direct reports.
  • Masters degree in Social Work or a related field AND minimum three (3) years of experience in social work, or a directly related field.

  • Licensed Clinical Social Worker (Georgia) required at hire
