Job Summary:

The Community Health Worker (CHW) will be Healthy Neighbor's frontline presence in the community. The CHW will be: working to improve patient health by providing social support, advocacy, and resource navigation; conducting health and wellbeing check-ins at clinics, community sites, patient homes and remotely by virtual visits and phone calls; connecting patients with follow-up clinic appointments, social resources, reinforce diabetes and hypertension health education and collaborate with patients to identify barriers to chronic disease management and work toward solutions; building strong relationships with patients and their families.

Position Responsibilities:

Perform needs assessments and enroll patients in Healthy Neighbor by meeting with prospective patients in-clinic, at community sites & through remote outreach.

Assist patients with their medical appointments - making appointments, sending reminders, coordinating transportation and support patients as needed. Refer patients to social services and community resources & follow up to ensure patient access to these supportive resources.

Provide patients with health education and coaching to help them manage their diabetes and/or hypertension.

Identify opportunities to improve Healthy Neighbor and work together with the care team to implement changes.

Position Qualifications Required:

Required Experience:

Demonstrated interest in community, social justice, public health and/or local economies.

Must display ease and comfort when building relationships with people of different backgrounds, abilities, opinions and perceptions & demonstrate the ability to treat all people with equity and respect.

Ability to travel to multiple work sites daily, using your own means of transportation. Valid driver's license required if driving own vehicle.

Available some evenings and weekends, depending on patient availability.

Bi-lingual (Spanish) strongly preferred.

Prior experience working as a community health worker (CHW) or community-based patient advocacy, engagement or resource navigation preferred.

Required Education:

High school diploma or GED.

Training / Certification / Licensure:

Community Health Worker certification preferred. Lay navigation training certification will be considered as an alternative to CHW certification.
