Pay: $50 per visit-$125 per visit

SOC- $100.00 per visit with an extra $25 bonus if notes are completed within 24hrs

JOB SUMMARY: The Pediatric Registered Nurse plans, organizes and directs home care services and is experienced in pediatric nursing from birth through late teens, with emphasis on community health education/experience.

RESPONSIBILITIES:

  • Completes comprehensive assessments of patient and family to determine home care needs at regular established timelines. Provides a complete physical assessment and history of current and previous illness(es).

  • Regularly re-evaluates patient nursing needs.

  • Initiates the plan of care and makes necessary revisions as patient status and needs change.

  • Develops the plan of care with patient’s physician; establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and family in the planning process.

  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician

  • Counsels the patient and family in meeting nursing and related needs.

  • Provides health care instructions to the patient/family as appropriate per assessment and plan.

  • Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient

  • Knowledge of high-tech nursing skills as applicable which is not limited to indwelling/straight catheter insertion/removal, nephrostomy care, ostomy care, plural and abdominal catheter drainage care, infusion services, g-tubes, and negative pressure wound therapy.

  • Working knowledge of all infection control procedures and safety precautions.

  • Knowledge of the nurse's scope of practice and ability to apply professional theory, practice and procedure showing appropriate application of the nursing process.

  • Observe ethical and legal guidelines for safeguarding confidentiality of patients.

  • Assess patient needs and physical status each skilled visit through health data assessment and patient interview. Re-evaluate patient needs through physical

  • assessment, response to therapy and any updated physician’s orders. Inform physician of any changes in patient condition, response to care, progress etc. Obtain and clarify orders for Plan of Treatment revisions.

  • Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Completes And submits all documentation in accordance with policy and procedures.

  • Communicates with the physician regarding the patient’s needs and reports and changes in the patient’s condition; obtains/receives physician’s orders as required.

  • Communicates with other health related persons to coordinate care plan.

  • Participates in on-call duties as defined by the on-call policy.

  • Ensures that arrangements for equipment and other necessary supplies and services are available.

  • Instructs, supervises and evaluates home health aide/personal care aide care provided every 14 days or as stipulated by law and regulation.

  • Instructs, supervises and evaluated LPN/LVN care provided at least every 30-days.

  • Exercises professional management. And coordinates care between and among involved parties in home, community providers and pediatric tertiary specialists.

QUALIFICATIONS:

  • Bachelor's degree of Nursing from an accredited college/university

  • Current active and unrestricted nursing license

  • Pediatric care experience in the acute and/or home care environment

  • Valid driver’s license

  • Current certification in Basic Life Support for Health Care Professionals.

  • Minimum of 1 year of recent acute or home care experience, preferably in an institutional setting.

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.

  • Basic knowledge of computer operating systems and software applications to communicate and document care.
