Per Diem On-Call Registered Nurse (RN) – Home Health

Evening On-Call | Somerset, Union, Middlesex & Monmouth Counties, NJ

Make a Difference When Patients Need It Most.

BAYADA Home Health Care is seeking an experienced Per Diem On-Call Registered Nurse (RN) to provide evening home health coverage for adult and geriatric patients throughout Somerset, Union, Middlesex, and Monmouth Counties.

In this flexible on-call role, you'll provide in-home skilled nursing visits, complete Start of Care admissions as needed, and deliver exceptional patient-centered care while being supported by an experienced clinical team.

Schedule

  • Monday through Thursday
  • 5:00 PM – 11:00 PM
  • On-call during scheduled hours
  • Conduct in-person home visits as needed
  • Complete approximately one Start of Care (SOC) visit per shift
  • Receive both an on-call stipend and visit-based compensation

Responsibilities

As a Per Diem On-Call RN, you will:

  • Respond to after-hours patient needs within your assigned coverage area
  • Complete Start of Care (SOC) admissions as needed
  • Perform comprehensive patient assessments and complete OASIS documentation
  • Deliver skilled nursing care, including medication administration, wound care, treatments, and other prescribed interventions
  • Monitor patient conditions and communicate changes to the Clinical or Client Services Manager
  • Follow physician orders and ensure timely, accurate documentation
  • Coordinate care with physicians, caregivers, and the interdisciplinary team
  • Document all patient care using an electronic medical record (EMR) system

Qualifications

  • Current Registered Nurse (RN) license in the State of New Jersey
  • Minimum of one year of recent, verifiable Home Health nursing experience
  • Experience completing OASIS documentation and Start of Care visits required
  • Strong assessment, clinical decision-making, and communication skills
  • Comfortable using electronic medical records (EMR); Homecare Homebase (HCHB) experience is a plus
  • Reliable transportation and ability to travel throughout Somerset, Union, Middlesex, and Monmouth Counties

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation:

$65.00-$75.00 per point
