Description Shift: 5x8/DAYS (8:30A-5P: Alternating Weekends), Length: 13 weeks, GT: 40, Start: 6/15/26 or sooner, POSITION REQUIREMENTS: at least 2 years of Home Health experience, ALL CANDIDATES MUST HAVE HOMECARE HOMEBASE/OASIS EXPERIENCE WITHIN THE LAST YEAR TO BE CONSIDERED, Experience with Wellsky EMR and wound care would be a bonus, Experience with on call / triage, Reliable and dependable, excellent time management skills, Professional, Customer service excellence, compassion and caring, Licensure in hand at time of submission. What is Health Care Connectors on call schedule and expectation for this assignment? - On- call would be one to two weeks a month- from 530pm- 830 am weekdays. It entails phone triage, very rare does it require a home visit. 1- 2 weeks a month.
