Job description
Role and Responsibilities
Administer direct skilled nursing care to patients, in accordance with physician's orders.
Essential Functions
- Provide hands-on, skilled nursing care in accordance with physician's orders.
- Provide health education to the patient and family.
- Assess patient response to the plan of care interventions and communicate changes to the case manager.
- Collaborate and share findings with the multidisciplinary care team.
- Document those services rendered to the patient, note changes in the patient's condition and/or family and home situations.
- Document communication with case manager and physician.
- Meet visit productivity requirements and conduct other home visits (SOC/ROC, recertifications, discharges) as requested by clinical manager.
- Other duties as assigned.
Education Requirements
- Current, valid RN license issued by NYS Department of Education.
- Community health experience preferred.
- Valid and unexpired NYS Driver's License.
- Current CPR certification.
- Strong communication and documentation skills.
- Critical thinking skills, and ability to function autonomously.
- Clinical Competency of nursing skills.
- Works as team player and collaborates with the patient care team.
The RN Procedure Nurse is primarily in a non-office setting and may be exposed to outdoor conditions.
The working conditions are classified as medium work:
