Job Title: Travel RN - Home Health

Schedule: 40 hours/week

On-Call/Weekend Requirement: Yes
Mileage Reimbursement: Mileage is paid from the first patient to the last patient.
Key Responsibilities:
- Perform 30 productivity points per week by completing Start of Care (SOC), admissions, routine skilled nursing visits, recertifications, resumptions of care, and discharge assessments.
- Provide comprehensive home health care, accurate OASIS documentation, and timely charting in compliance with Medicare and agency standards across the Newburgh, NY territory.
- Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.
- Provide skilled nursing interventions, medication and disease management, wound care, and patient/caregiver education.
- Participate in the on-call rotation and ensure high-quality, patient-centered care.
Must Have Qualifications/Experience:
- HCHB experience is a must.
- 1-2 years of home health experience.
