Intake Registered Nurse (RN)

Salary: $100,000 annually Location: Brooklyn, NY (Office-Based)

Position Summary

We are seeking an experienced Intake Registered Nurse (RN) for an office-based position with a Certified Home Health Agency (CHHA) in Brooklyn. The Intake RN is responsible for coordinating referrals, processing new home care admissions, verifying eligibility, and ensuring a seamless transition of patients into care. This role requires excellent communication, attention to detail, and experience working within a fast-paced home care environment.

Responsibilities
  • Receive, review, and process referrals for appropriate home care services.
  • Maintain communication with referral sources, physicians, and internal departments.
  • Ensure timely and accurate documentation within the agency's EMR system.
  • Verify clinical and insurance eligibility to support patient admissions.
  • Provide exceptional customer service while promoting the agency's mission and values.
Requirements
  • Education: Graduate of an accredited School of Nursing.
  • Licenses & Certifications: Current New York State Registered Nurse (RN) License (Required).
