The Organization
San Gabriel/Pomona Regional Center (SG/PRC) is a private, nonprofit agency contracted with the California Department of Developmental Services (DDS) to provide services and supports to individuals with intellectual and developmental disabilities. We proudly serve 30 cities across the Foothill, Pomona, and El Monte Health Districts within Los Angeles County. Our services span the full lifespan-from infants and toddlers, school-age children, and transition-age youth to adults and aging adults. Position Description
Reporting to the Manager of Pre-school Services V, the Service Coordinator provides comprehensive and compassionate case management to children ages 3 to 6 and their families. This role offers the opportunity to make a meaningful difference working collaboratively with families and community resources to ensure the individuals we serve receive high-quality service support and thrive.
Essential Job Functions
  • Assist children with developmental disabilities and their families in locating appropriate community services and programs.
  • Consult with the Manager, Client Services Staff, liaison, and specialist staff of the Regional Center to coordinate services for children served including medical, behavioral health services, psychological services, educational, vocational, recreational programs, and adaptive devices.
  • Explain available resources and services to individuals served and their families, ensuring they understand their options.
  • Collaborate with the team to assess and address the children's needs effectively.
  • Conduct in-person meetings with children, their families, and other relevant agencies to facilitate service planning and coordination.
  • Develop and implement individual Family Service Plans (IFSP) tailored to each child's unique needs.
  • Document service plan, meetings, and progress accurately and in a timely manner.
  • Document interim and annual reports within mandated timelines.
  • Initiates purchase of service (POS) through the Regional Center for clients who are ineligible for other sources of funding.
  • Present cases at specialized committees as appropriate.
  • Performs additional duties that support departmental and organizational goals.

Requirements
Employment Standards
  • Bachelor's degree in social work, psychology, human development, sociology, public health nursing, or a related field.
  • At least one year of experience working with individuals with developmental disabilities or a related field.
  • Master's degree in a related field can be substituted for the required experience.
  • Bilingual in Vietnamese, Mandarin, or Cantonese is preferred.

Knowledge and Abilities
  • Knowledge of developmental disabilities and community resources.
  • Strong interpersonal and communication skills (verbal and in writing).
  • Experience in developing and implementing individualized service plans and meeting strict document timelines.
  • Knowledge of behavior intervention techniques and best practices.
  • Ability to independently plan and schedule work.
  • Ability to develop individualized support plans and adapt strategies as needed.
  • Strong organizational skills and attention to detail for documentation and case management.
  • Ability to conduct in-person meetings and facilitate collaboration among various stakeholders.
  • Excellent computer skills in MS Office, including Word and Outlook.
  • Ability to multi-task, prioritize assignments, and meet deadlines with conflicting priorities.
  • Ability to provide a compassionate, culturally sensitive approach to working with diverse populations.

Other Essential Requirements
  • Service coordination is a community-based position that requires meeting with individuals served and their families in their home, in the community, or in our office location.
  • Frequent telephone, virtual, and out-of-office meetings are required.
  • A valid driver's license, reliable transportation and minimum liability insurance coverage is required.

Physical Demands & Work Environment
  • Work 60% of the time in an office environment and 40% in the community at meetings or home visits.
  • Ability to drive to and from meetings or home visits and walk from car to and from appointments.
  • Occasional walking and standing to attend and participate in office meetings and trainings.
  • Ability to sit for extended periods and operate a computer for report writing.
  • Clear verbal communication in person and by phone.
  • Occasional lifting, pushing, or pulling of items up to 25 pounds.
  • Adequate visual acuity to read documents, use computer monitor for prolonged periods, and review detailed information.
  • 60% of the work time occurs in a typical office setting with standard lighting, temperature, and moderate noise levels.
  • Employees are expected to follow safe and ergonomic work practices.
