At CenterLight Healthcare PACE, our mission is simple: help older adults live safely and independently in their communities while receiving the highest quality care possible.

We're seeking a dynamic healthcare leader to serve as our Center Service Director (CSD), overseeing interdisciplinary care delivery, center operations, participant outcomes, and clinical performance at one of our PACE centers.

This is an opportunity to lead a multidisciplinary team, influence strategic decision-making, and drive meaningful outcomes for a vulnerable and deserving population.

What You'll Do

Lead Care Delivery Operations

  • Oversee daily operations of the Interdisciplinary Team (IDT) and all center-based clinical services.
  • Ensure coordinated, participant-centered care across the Day Health Center, Diagnostic & Treatment Center, Home Care Services, and community settings.
  • Partner with clinical and operational leaders to improve participant outcomes, satisfaction, and quality performance.
  • Balance staffing resources and ensure effective team coverage across departments.

Drive Quality & Regulatory Excellence

  • Lead quality improvement initiatives and support CMS, NYSDOH, PACE, LHCSA, and Article 28 compliance efforts.
  • Oversee incident investigations, grievance resolution, and corrective action plans.
  • Ensure documentation, care plans, and participant records meet organizational and regulatory standards.
  • Lead survey readiness activities and regulatory audits.

Manage Performance & Outcomes

  • Monitor key performance indicators including hospital admissions, nursing home admissions, enrollment/disenrollment trends, utilization management, quality benchmarks, budget performance, and participant satisfaction.
  • Develop strategies that reduce avoidable hospitalizations and improve care transitions.
  • Promote effective utilization of PACE services and resources.

Lead & Develop Teams

  • Provide leadership, coaching, mentoring, and accountability for managers and interdisciplinary staff.
  • Foster a culture of collaboration, innovation, service excellence, and continuous improvement.
  • Support professional development and succession planning efforts.
  • Some flexibility and occasional weekend or on-call support may be required.

Required Qualifications

  • Bachelor's Degree in Healthcare Administration, Business Administration, Nursing, or a related field.
  • Minimum three (3) years of healthcare leadership experience in a management capacity.
  • Supervisory and team leadership experience.
  • Experience managing clinical operations, healthcare services, or interdisciplinary care teams.
  • Strong operational, analytical, and problem-solving skills.
  • Excellent communication and relationship-building abilities.
  • Customer service focused mindset.

Preferred Qualifications

  • Experience within PACE, managed care, long-term care, home care, or healthcare operations.
  • Experience serving frail, elderly, or medically complex populations.
  • Bilingual language skills.
