Aging-in-Place Consultant โ€“ Help LA Seniors Stay Safely at Home Company: All Seniors Foundation
Location: Los Angeles, CA About Us:

All Seniors Foundation is a 501(c)(3) non-profit committed to improving the quality of life for older adults across Los Angeles County. we provide medical care, in-home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status.

Role Overview:

As Aging-in-Place Consultant, you will lead home assessments that allow seniors to age safely in their own homes. You will identify fall risks, recommend modifications (grab bars, ramps, lighting), and coordinate installation with contractors and equipment vendors.

Key Responsibilities:
  • Conduct comprehensive home safety assessments using AOTA Home Modification framework
  • Identify fall hazards, accessibility barriers, and adaptive-equipment needs
  • Recommend modifications: grab bars, ramps, raised toilet seats, stair-glides, smart-home tech
  • Coordinate with contractors, CAPS specialists, and DME vendors on installation
  • Educate seniors and families on energy conservation, fall prevention, and home safety
  • Document assessments and follow up on completed modifications
Qualifications:
  • Occupational Therapist (OT) license required (CA OTR/L)
  • CAPS (Certified Aging-in-Place Specialist) certification preferred
  • 2+ years OT experience; home health or aging-in-place focus a plus
  • Strong assessment, communication, and project-management skills
  • Familiarity with ADA, AOTA, and California building accessibility codes
