Senior LTSS Service Care Manager (RN)

Location: Remote (Texas) – Must cover Bell, Bosque, Burleson, Comanche, Erath, Gillespie, Grimes, Lampasas, Llano, McLennan, Mills, San Saba & Washington Counties

Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Duration: 12 Months (Possible Extension)

Job Summary

Seeking an experienced RN to manage care for high-acuity LTSS members.

Responsibilities include conducting in-home assessments, developing care plans, coordinating services, educating members, ensuring regulatory compliance, and documenting care activities.

This is a field-based, member-facing role requiring travel within assigned counties.

Required Qualifications

  • Graduate of an Accredited School of Nursing
  • Active Texas RN License (Compact accepted)
  • 4–6 years of nursing/case management experience
  • Valid Driver's License
  • Ability to travel for member visits
