Job description

JOB SUMMARY:

The Resident Care Manager (RCM), Memory Care (MC) is a licensed nurse (LVN or RN) responsible for overseeing the care of residents, ensuring their safety, comfort, and well-being. The RCM coordinates staffing, resident care, and clinical services in collaboration with the Executive Director, Wellness Nurse, and the Health Services Director. They work closely with the Assisted Living (AL) RCM to ensure streamlined processes across both AL and MC, and serve as backup to the RCM, AL when needed. This role also communicates with physicians, families, and healthcare professionals regarding resident care and clinical concerns.

ESSENTIAL JOB FUNCTIONS:

  • Oversees MC resident care plans; monitors changes in health, cognition, or behavior, and ensures proper documentation.
  • Implements person-centered care strategies for residents.
  • Monitors and manages staffing schedules to ensure resident needs are met; responds to staff call-offs and coverage needs.
  • Serves as primary clinical contact for families, physicians, and other healthcare providers regarding memory care residents.
  • Collaborates with the Wellness Nurse and RCM AL to ensure consistent processes and high-quality care across the community.
  • Participates in resident assessments, care conferences, and interdisciplinary team meetings.
  • Assists new residents and families with acclimating to the AL environment.
  • Ensures compliance with RCFE and MC regulations, including resident assessments, 602s, and infection control protocols.
  • Provides training, orientation, and ongoing development for MC staff.
  • Manages medication administration and oversees ordering and inventory of medical supplies.
  • Supports behavioral management strategies and intervenes appropriately during resident incidents.
  • Attends and participates in required trainings, in-service programs, and change-of-shift meetings.
  • Serves as backup for the RCM AL; assists with AL resident care oversight in their absence.
  • Perform other duties as assigned to support resident safety, quality care, and community operations.

QUALIFICATIONS:

  • Current LVN or RN license in California required.
  • Two years’ experience in an RCFE, MC or AL, providing direct resident care and/or clinical oversight.
  • Strong knowledge of dementia care principles, behavioral interventions, and person-centered care.
  • Ability to communicate effectively with residents, families, staff, & in sensitive or difficult situations.
  • Successful completion of ACC’s medication training program within 60 days of hire.
  • Obtain and maintain valid First Aid and CPR certification within 30 days of hire.
  • Ability to work flexible hours, including evenings or weekends as needed.
  • Proficiency in Microsoft Office and electronic health records (e.g., PointClickCare / PCC).
