Job description

MEDICAL RECORDS CLERK
San Diego, CA

Due to growth, we are recruiting for an experienced Medical Records Specialist in the home health setting to join our team. If you are driven, enthusiastic, knowledgeable in this specialty, and seeking a great career opportunity, we may have the perfect role for you.

This position will be based in our San Diego, CA office and will be responsible for supporting and managing medical records functions for both California offices.

The candidate must also be willing to use and/or be trained on a multi-line phone system to back up reception as needed.


Duties Include:
  • Scan paperwork (labs, physician orders, miscellaneous documents) into the electronic medical record (EMR) and label documents appropriately

  • Follow up on outstanding 485s and interim orders with physicians (calls, re-faxes, etc., as needed)

  • Print and mail or deliver orders to physicians on the “do not fax” list

  • Check received 485s and interim orders into software and save copies in the EMR

  • Fax labs to physicians as needed

  • Fax patient information to physicians and other authorized parties as needed

  • Submit Face-to-Face (F2F) requests to physicians and referral sources as needed

  • Follow up on F2F documentation

  • Check received F2F documentation into software once approved by the Director and save copies in the EMR

  • Forward paper visit notes to the logging department for processing

  • Research End of Episode (EOE) reports daily and report findings to the billing department

  • Prepare and send daily reports as requested

  • Primary responsibility: Companywide follow-up on outstanding medical record orders and/or deficiencies

  • Assist California offices as needed or directed

Education:
  • High school diploma or equivalent required

  • Medical records certificate preferred


Experience:
  • Minimum of one (1) year of prior filing experience and experience contacting physicians for orders required

  • Minimum of five (5) years of medical records experience required; home health experience strongly preferred

  • Demonstrated ability to multitask in a fast-paced environment

  • Experience working with high-volume charts and file tracking required

  • Working knowledge of HIPAA required

  • Strong computer skills required


Job Requirements:
  • Ability to multitask and complete assigned projects and reports in a timely manner

  • Minimum of three (3) years of medical records experience in a high-volume, fast-paced medical setting
