Job description

Avidd Community Services of New Jersey, Inc proudly serves people with intellectual and developmental disabilities in Morris, Bergen, and Somerset Counties. Our residential programs are homes that are settled in various communities throughout suburban North Jersey. Each home is custom suited to its residents, their abilities, needs and personalities. Avidd strongly believes in fostering independence and teaching life skills.
We are currently hiring a Licensed Practical Nurse (LPN) for our residential group home serving individuals with physical and intellectual disabilities in Hackettstown, NJ.
Shift Schedule: Monday- Friday 7am-3pm (subject to change)

Role of a Licensed Practical Nurse (LPN):
  • Provide nursing care to all individuals that require specialized protocols that require intervention from an LPN such as wound care, feeding tubes, etc.
  • Provide independence skill training to group home residents under the direction of the Residence Manager.
  • Complete and review all communication and client logs in the program, including electronic records. These include: SCOMM messages, ISP Data, T-Logs, ABC sheets, Incident reports, Medication and Medical supply delivery, PRN/OTC administration, and any other medical or program log as assigned.
  • Clean, organize and disinfect the program according to OSHA/DDD guidelines.
  • Provide transportation to all individuals in a safe manner, in a facility provided program vehicle.
  • Administer medication to individuals being served and document accurately on Medication Administration Records.
  • Reorder and discontinue individuals’ medications according to doctors’ orders and document accurately on MARs and communication logs.
  • Supervise self-help skills of individuals; if necessary, provide least intrusive assistance.
  • Accompany and advocate for clients on medical appointments, ensure that scripts are written according to DDD standards, and information is documented on all communication logs.
  • Assist individuals in nutritional food preparation and adhering to specialized diets.
  • Attend to all personal care as required, including toileting, dental hygiene (brushing and dental flossing if necessary), bathing etc.; reposition, lift, and transfer any individuals who utilize wheelchairs.
  • Assist with all feedings or provide supervision during mealtimes as required.
  • Complete accurate documentation in the Electronic Health Record (Therap) on every shift.
  • Provide necessary First Aid and CPR to individuals in need.

Requirements:
  • High School Diploma / GED required.
  • Valid Licensed Practical Nurse in NJ or verification of an out-of-state temporary LPN through NJ’s Division of Consumer Affairs.
  • Knowledge and understanding of the IDD community and services.
  • Valid driver's license and willingness to undergo pre-employment requirements.
