Case Manager - School-Based
From participating in intakes to determine consumer needs and capabilities to serving as primary liaison between HCS and the school regarding HCS services and service provisions, this position gives you the opportunity for meaningful, hands-on work alongside community partners for greater continuity of care.
Position Specifics
Location: Washington County VA or City of Bristol, VA
Status: Full-time, Non-exempt, Monday-Friday
Compensation: $18.50 - $20.00 - Commensurate with education/experience
Education: Bachelor's degree in Human Services or related field
Experience: Registered as a Qualified Mental Health Professional and experience in a school setting preferred.
