Description:

The Intake Coordinator works cross-functionally within the Member Engagement and Community Partnerships departments. Serving as a primary point of contact for prospective members and referral sources, they also guide new members through the process of beginning services. This role is responsible for managing the intake process from initial inquiry through service initiation, ensuring a smooth, timely, and compassionate onboarding experience. The Intake Coordinator gathers member and caregiver information, verifies eligibility, educates potential members about the programs, and collaborates with internal teams to convert referrals to admissions.


Key Responsibilities:

  • Respond promptly to incoming inquiries from clients, families, and referral sources via phone, email, and in-person communication
  • Conduct initial intake screenings to gather client demographics, service needs, preferences, and schedules
  • Explain available non-medical personal care services clearly and professionally
  • Verify service eligibility
  • Maintain accurate and up-to-date client notes in company software
  • Collaborate with HR and care coordination teams to ensure timely start of services
  • Track referral progress of each lead
  • Follow up with prospective clients and care coordinators to convert referrals into admissions
  • Ensure compliance with company policies, state regulations, and documentation standards

Requirements:
