Job Summary

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual clients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for clients and are able to observe and communicate effectively in your client's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems. You will also be responsible for training the caregivers.

Responsibilities

Coordinate with the client's physician to assess clients and implement a care plan

Orientate and train the caregivers that are responsible for daily activities with the clients

Document any changes in client conditions and report changes with their assigned physician

Maintain accurate records of client care and discuss observations with the physician

Coordinate classes to teach the caregivers daily activities

Qualifications

Currently licensed Registered Nurse (RN) in State of employment

You must have one year or more experience in home health care or hospice care

Excellent interpersonal and client care skills

Valid driver’s license

First aid/CPR certification required

Valid Registered Nursing license
