As we continue to expand our clinical programs to meet increasing community needs, we are seeking RN Case Managers to join our teams in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties!

At Nascentia Health, you’ll provide skilled, compassionate care to adults with chronic, acute, or complex medical needs in the comfort of their homes, helping patients maintain independence, improve quality of life, and feel supported in their own environment. We believe in listening to our nurses, supporting them, and setting them up for success every day. Home visits are scheduled Monday–Friday, 8:00 AM–4:30 PM, with no on-call requirement, and a one weekend a month rotation. This Full-Time position offers flexible scheduling and a full benefits package!

What You’ll Do as an Adult RN Case Manager

Provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. You’ll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.

Your responsibilities include:

  • Deliver safe, patient-centered nursing care in the home
  • Serve as the primary case manager, coordinating care with the interdisciplinary team
  • Develop and implement individualized plans of care
  • Provide patient and caregiver education, support, and disease management teaching
  • Ensure accurate, timely documentation (documentation time built into the home visit)
  • Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care
  • Maintain compliance with all infection control and regulatory standards
  • Stay current with clinical best practices

What We’re Looking For!

  • Current NYS Registered Nurse license (from an accredited program)
  • BSN preferred (tuition reimbursement available!)
  • Valid driver’s license, auto insurance, and reliable vehicle
  • Self-directed, organized, and comfortable working independently
  • Adult medical-surgical, home care, case management, or chronic disease management experience a plus
  • Strong communication and computer skills

Physical Requirements:

  • Ability to sit or stand for long periods during work day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
