About the Job
Job Summary:
MedStar Health is looking for a Hospital Responder to join our team! The ideal candidate will have personal knowledge of the target population, such as at-risk youth and adults, cultural competency, knowledge of local resources and system navigation and advocacy and community capacity building skills.
The Hospital Responder will be responsible for directing intervention activities of the Safe Streets Hospital Program and supporting the work of trauma-informed and culturally relevant programming in coordination with hospital and external stakeholders.

Primary Duties:
  • Implements direct intervention activities of the Safe Streets Hospital Responder program, including supporting work of emergency department team. Facilitates linkage of injured individuals to appropriate Safe Streets community sites for those residing in Safe Street communities, community resources and/or other programs that assist the participant develop resources and establish relationship with positive social support networks.

  • Meets interested patients and loved ones following a violent injury for follow up assistance during their recovery from injuries. Acts as participant advocate, coordinates, and makes referrals for services. Monitors and tracks services provided and must complete all participant encounters and contracts made on behalf of participants. Coordinates and monitors services, including comprehensive tracking of participant activities in relation to action plan.

  • Collaborates with other staff to form action plans and strategies to help resolve conflicts and prevent shootings. Participates in regular staff meetings conducted by Safe Streets Baltimore and the hospital. Maintains records and participant related files, drafts reports, composes letters, and completes electronic and written documentation, as appropriate.

Qualifications:
  • High School Diploma or GED.

  • Training in crisis intervention.

  • Participation with a community organization, verified by personal or professional reference with personal knowledge of this fact preferred.
