Crescent Hospice is currently hiring a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the Anderon, Oconee, & PickensArea!
Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing and/or admissions in a Patient's home or in a health care facility. This position works with the intake team, referring physician, and other referral sources to promote the hospice philosophy of care.
SCHEDULE: Monday - Friday, 11:00am- 7:30pm
TERRITORY: Anderson, Oconee, Pickens

Responsibilities
  • Reviews clinical referral information and responds to request and inquires as appropriate. Collaborates with Clinical Director regarding the eligibility and appropriateness of client for hospice services.
  • Provides information, pre-assessment, and admission services to patients who have been referred to the hospice; conducting pre-assessments visits in the home or healthcare facility as necessary.
  • Communicates with the Intake Department, the referring physician and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
  • Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient's plan of care to meet established goals.
  • Participates in On-Call duties as assigned and defined by Clinical Director and Program Director.

Qualifications
  • Bachelor's degree in nursing or health care preferred.
  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Hospice/home health experience preferred but not required.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

$70,000 - $80,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
