Position Overview Infinite Medical P.C. is seeking an on-site, detail-oriented Licensed Nurse (RN or LPN) to serve as a Nursing Care Coordinator. Operating strategically in an administrative role, you will review and verify chart documentation to ensure compliance and support seamless continuity of care for our patient population.

Key Responsibilities

  • Compliance & Records Review: Evaluate patient records and Review Visit Logs (RVLs) to ensure clinical accuracy and strict adherence to CMS and internal operational guidelines.
  • Quality Assurance: Identify documentation discrepancies, generate clinical corrections, and submit suggestions for resolution.
  • Cross-Functional Communication: Coordinate with HR, Operations, and Clinical teams to resolve inquiries and escalate technical issues promptly.
  • Professional Development: Participate in mandatory operational and educational meetings, complete post-training surveys, and cross-train in expanding care management verticals.
  • Systems Utilization: Maintain proficiency across primary digital platforms, including Emsow, PointClickCare, Sigma/Matrix, Reliable/Visual, Yardi-One and Google Workspace.

Core Knowledge & Competencies

  • Clinical Documentation & Chart Review: Review charts for completeness, accuracy, regulatory compliance, and QA standards.
  • Care Management: Expert understanding of chronic disease management, care planning, care transitions, and patient engagement.
  • LTC & SNF Operations: Familiarity with Skilled Nursing Facility workflows, common geriatric diagnoses, and interdisciplinary care.
  • EMR Systems: Hands-on experience with PointClickCare (PCC), MatrixCare, SigmaCare, or similar healthcare platforms.
  • Medical Literacy: Strong grasp of medical terminology, pharmacology, diagnoses, medications, and treatments.
  • Execution & Communication: Exceptional attention to detail to find discrepancies, strict time management to meet daily productivity goals, and professional communication skills.
Preferred Knowledge
  • CMS guidelines for chronic care documentation.
  • ICD-10 diagnosis coding fundamentals.
  • Review Visit Log (RVL) completion and clinical review principles.
  • Microsoft Office and Google Workspace proficiency.

Position Requirements

  • Licensure: Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
  • Clinical Expertise: Strong foundational knowledge of geriatric care and chronic medical conditions.
  • Schedule: Commitment to a full-time, on-site, standardized schedule: Monday through Friday, 9:00 AM – 5:30 PM.
