Care Coordinator

Care Runners Home Care Services, Inc. is seeking a dedicated Care Coordinator to join our team in McDonough, GA. The ideal candidate will have experience working with clients with developmental disabilities and a strong person-centered focus to support our clients effectively. This role involves coordinating care plans and services tailored to individual needs, ensuring quality and compassionate care for our clients.

Responsibilities
  • Coordinate and oversee care plans for DBHDD clients receiving home care services, ensuring they meet individual needs and preferences.
  • Collaborate closely with clients, families, and healthcare providers to deliver person-centered care.
  • Monitor client progress and adjust care plans as necessary to maintain high standards of care and client satisfaction.
  • Maintain accurate and timely documentation in compliance with company policies and regulatory requirements.
  • Serve as a liaison between clients, care staff, and external agencies to facilitate communication and service delivery.
