Key Responsibilities

• Conduct thorough assessments of patients' emotional, psychological, and social needs to create tailored treatment plans.
• Implement evidence-based therapeutic interventions to help patients cope with end-of-life challenges, including grief, anxiety, and depression.
• Maintain accurate and confidential records of patient interactions and treatment plans in compliance with regulatory standards.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare professionals, to ensure coordinated patient care.
• Participate in regular team meetings to discuss patient progress, share insights, and refine treatment approaches.
• Stay current with best practices, research, and emerging trends in the field of social work and hospice care.

Patient Population / Client Focus

The patient population at our Hospice Center includes individuals facing terminal illnesses and their families. As an LCSW, you will work closely with patients experiencing significant emotional distress, helping them navigate through their feelings about dying, impending loss, and unresolved issues. You will also support caregivers and families in coping with the demands of providing end-of-life care, ensuring they feel empowered and supported throughout the process.

Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid LCSW (Licensed Clinical Social Worker) license in the state of Tennessee.
• Strong verbal and written communication skills, with the ability to build rapport and trust with diverse populations.
• Exceptional problem-solving abilities and a commitment to patient-centered care.
• Ability to thrive in a remote and collaborative work environment.

Preferred Experience

• Experience working with patients in hospice or palliative care settings.
• Familiarity with trauma-informed care principles.
• Previous experience using electronic health record (EHR) systems for documentation.
• Knowledge of Medicare regulations concerning hospice care.

Compensation and Benefits
